Who Can Refer?
We accept referrals from a range of sources and welcome self-referrals. No GP referral is required to access occupational therapy through the NDIS.
Support Coordinators
NDIS support coordinators can refer participants for capacity building and therapeutic supports.
Plan Managers
Plan managers can connect their participants with our OT services and assist with funding coordination.
Self-Referral
You can refer yourself or a family member directly. Simply fill out the form above and we will guide you through the process.

Initial Consultation
We will have an initial conversation to understand the individual's needs, goals, and funding arrangements.

Assessment
Our therapist visits the participant at home to conduct a thorough assessment of their functional abilities and environment.

We develop a personalised treatment plan with clear goals and begin working towards meaningful outcomes.
