An FCA describes how disability affects everyday activities and what support the person needs. The report can bring together information from the participant, relevant observations and other evidence. It should explain the reasons for its recommendations in language the participant and decision-maker can follow.

An assessment grounded in daily life

An in-home visit allows the OT to consider the spaces and routines used regularly. An entrance step, the layout of a bathroom or the way kitchen items are stored may help explain why a task is difficult. The same activity can be manageable in one setting and harder in another.

Tell the therapist which routines matter most to you. These might include preparing food, personal care, leaving home for an appointment or managing a daily sequence. Discuss the support you already receive, including the help that family provides without a formal roster.

  • Communication, access or support-person requirements for the visit.

The therapist can discuss which documents are necessary. Avoid delaying the enquiry because you do not yet have every report collected.

What the report can address

The agreed scope may include a description of current function, the effect of the environment, assistance required and recommendations for practical supports. It should distinguish between what was observed, what was reported and what still needs further investigation.

A general FCA is not automatically a complete wheelchair prescription, a detailed home modification design or a housing assessment. If one of those is the main question, explain it at referral so the appropriate service can be discussed.

Costs and report timing

Ask for a written estimate covering professional time, report preparation and any relevant travel. Tell us about a funding or submission deadline before booking. Appointment and report timing need to be confirmed for the specific assessment; this page does not promise a fixed turnaround.

A well-prepared report provides evidence, but the NDIS or another funding body makes its own decision. An assessment cannot guarantee approval of a support or a particular amount of funding.

Can a family member join the assessment?

Discuss this with the participant and therapist. A family member can help explain daily support, while the participant's voice, consent and preferences remain central.

Do I have to complete every task during the visit?

No one should attempt an unsafe activity to demonstrate difficulty. Tell the therapist about the concern so they can agree on an appropriate way to gather information.

Can the OT use previous reports?

Relevant recent evidence can be useful. The therapist still needs to decide what information is adequate for the current purpose and whether circumstances have changed.
